Licensed Vocational Nurse - Sherwood Oaks Post Acute (Healthcare)
PM Shift: 3pm-11pm
Direct and Supervise Nursing Staff Oversee day-to-day functions of nursing assistants, ensure compliance with policies, conduct evaluations, and provide leadership to maintain quality care.
Medication Administration and Documentation Prepare, administer, and document medications per physician orders, maintain accurate narcotic records, and report discrepancies.
Resident Care and Monitoring Deliver direct nursing care, monitor residents' physical/emotional condition, respond to incidents, and ensure care aligns with individualized plans.
Admissions, Transfers, and Discharges Coordinate and perform resident admissions, discharges, and transfers, including related documentation and communication with families.
Care Plan Oversight Review and implement care plans daily, ensure nursing assistants are informed, and collaborate on MDS assessments and care planning updates.
Charting and Reporting Accurately document in medical records, complete incident reports, transcribe physician orders, and communicate resident status updates.
Infection Control and Safety Ensure infection control protocols and safety regulations are followed, maintain clean and sanitary conditions, and monitor staff compliance.
Training and Staff Development Assist in new employee orientation, provide in-service education, and participate in training programs for continuing education and compliance.
Communication and Collaboration Maintain strong communication with residents, families, interdisciplinary teams, and external agencies to support coordinated care.
Emergency and Incident Response Respond to occupational exposures, emergencies, and code situations appropriately, and assist in evacuations or disaster preparedness as needed.
