We are seeking a Population Health Coordinator to:
Education:
Minimum – Bachelor's degree in Health Care Administration, Public Health, Social Work, Business Administration, Economics, Industrial Engineering or related field. Four years of relevant process improvement and project coordination experience may be considered in lieu of degree in addition to the experience below.
Work Experience:
Minimum – Two (2) years of process improvement and project coordination experience
Preferred – Two (2) years of process improvement and project coordination experience in a health care setting
Required Skills, Knowledge, and Abilities:
Physical Requirements:
Sedentary: Ability to lift up to 10 pounds maximum and occasionally lifting and/or carrying such articles as dockets, ledgers and small tools. Although a sedentary job is defined as one, which involves sitting, a certain amount of walking and standing is often necessary in carrying out job duties. Jobs are sedentary

Work Schedule:
This is a full-time, 1.0 FTE position that is scheduled to work Monday through Friday, 8:00 am – 5:00 pm. Hours may vary based upon the operational needs of the department.
