Nurse Practitioner - Home Primary Care (Healthcare)

AuthoraCare Collective is currently seeking a Nurse Practitioner (Home Primary Care). This is a full-time position; Monday - Friday 8:00am - 5:00pm (office base will be High Point) with on-call responsibility.
CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

The Nurse Practitioner (NP) Primary Care is responsible for delivering comprehensive primary care, palliative care, and chronic disease management to patients in their homes. The Nurse Practitioner conducting physical examinations, diagnosing and treating illnesses, managing chronic conditions, engaging in advanced care planning discussions, and coordinating care with other healthcare providers. The NP focuses on health promotion, disease prevention, and patient education to ensure continuous and holistic care for home-bound patients. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.

The Nurse Practitioner is responsible for building and maintaining therapeutic relationships with patients and caregivers, including providing emotional support and education, is a key aspect of this role. The NP will collaborate closely with the interdisciplinary team and effectively communicate, both verbally and in writing, with team members, patients, caregivers, physicians, pharmacists, and other healthcare professionals and community service organizations. Participation in on-call coverage for the Integrated Health Services population is required as assigned.
