ECM Program Supervisor (Information Technology)

As our Enhanced Care Management (ECM) Program Supervisor, you'll lead with purpose, guiding a dedicated care team that supports individuals facing complex medical, behavioral, and social challenges.
This role is about more than oversight--it's about championing compassionate, person-centered care. You'll balance program operations, staff development, and compliance while ensuring every client receives the respect, advocacy, and holistic support they deserve.

NOTE: THIS POSITION IS BASED OUT OF MERCED/MODESTO AREA.
What You'll Do
Lead & Support the Team

Provide day-to-day supervision, coaching, and mentorship to ECM Care Managers, Community Health Workers, and support staff.

Monitor workloads, documentation, and case progress to keep the team on track.

Build a positive, inclusive culture rooted in growth, accountability, and heart.

Identify training needs and help your team strengthen their skills.

Ensure Quality & Compliance

Oversee program delivery in alignment with DHCS and managed care plan (MCP) requirements.

Track KPIs like outreach success, engagement, and timely care plans.

Audit records to ensure quality, compliance, and client-centered approaches.

Participate in audits, reporting, and quality reviews.

Champion Client Outcomes

Make sure clients are properly enrolled, assessed, and supported with tailored care plans.

Step in to resolve escalated client issues with professionalism and empathy.

Elevate the client voice--advocating for system improvements based on feedback and outcomes.

Collaborate & Communicate

Serve as the connector between staff, leadership, MCP partners, and community providers.

Participate in case conferences, staff meetings, and collaborative initiatives.

Foster clear, respectful, and timely communication at all levels.

What We're Looking For
Minimum Requirements:

Bachelor's degree in Social Work, Nursing, Public Health, Human Services, or related field (Master's preferred).

2+ years in a supervisory or leadership role in healthcare, behavioral health, or social services.

Experience with Medi-Cal populations, care coordination, or case management.

Knowledge of the ECM model, managed care systems, and California health programs.

Preferred Qualifications:

Bilingual (English/Spanish or another threshold language).

Familiarity with local community-based resources.

Training or experience in Trauma-Informed Care and Motivational Interviewing.
