Summary
We are seeking a Director of Intake to lead our efforts in providing high-quality care and support to our Skilled Nursing, Managed Care and Long-Term Care residents. This role is pivotal in overseeing case management and admissions operations, ensuring that our residents receive the best possible care tailored to their individual needs from admission to discharge at our communities. The position is based in Mayfield Heights, Ohio and plays a critical role in upholding our commitment to excellence.
This is a great opportunity for someone experienced case management and admissions in a Long-Term Care setting, specifically those that have multiple years' experience or prior Director experience leading a team of case managers or admissions team members. Prior experience with admissions or Case Management in Skilled Nursing facilities, hospital systems, or assisted livings is a plus! This In- Office position will focus on the financial authorization side of admissions and the case management of each resident in our care.
Starting negotiable rate for this position is $60,000 a year

Responsibilities

Oversee the development and implementation of centralized admissions and case management programs for residents.
Manage a team of case managers and support staff, providing guidance and supervision.
Ensure compliance with healthcare regulations and organizational policies.

Has knowledge of submitting for authorization for entry into Skilled Nursing

Monitor budgets related to case management operations and program development.
Evaluate program effectiveness and implement improvements as necessary.
Collaborate with healthcare professionals to coordinate comprehensive care plans.
